ELM Pediatric Dentistry also accepts the following Medicaid plans

(restricted to children ages 8 and below)

- Aetna Better Health

- Humana Healthy Horizons

- Wellcare 

- Molina Health (Passport)

- Kentucky State Medicaid

Anyone using one of the above Medicaid plans will require a referral from the patient's current dentist or pediatrician. A copy of our referral forms can be found below.  No age restriction for children who require sedation or have special health care needs.
